A Clinicao- athological Review of Cavitary Lesions in Pulmonary Cancer

Abstract
The author reviewed the eight cases of cavitary primary pulmonary cancer clinico-pathologically
and the following results were obtained.
1. Radiologically, cavitary lesions were seen in 8(7.7%) of 104 cases of primary pulmonary
cancer and they were 16.7% in squamous cell type and 9.5% in undifferentiated cell type. There
was no cavitray lesion in the adenocancer.
2. Histo-pathologically, 6 cases were squamous cell type and 2 cases were undifferentiated cell
cancer in the 8 cases of cavitary pulmonary cancer.
3. Repiratory symptom was presented in all of the cavitary cases and the main symptom was
hemoptysis or bloody sputum.
4. Cavitary lesions were seen in the lower lobe in 5 out of 8 cases of cavitary primary pulmo
nary cancer and only one case was seen in the upper lobe.
5. The cavitary lesions were about 3 to 30§¯. in diameter and the fiant cavities were revealed
that round in shape and thin wall with horizontal fluid levels.
6. Previous treatments for pulmonary abscess or TB were done in all cases for relatively long
period and also 2 cases were mis-diagnosed as pulmonary abscess preoperatively.
7. Lobectomy was done in 5 out of 8 cases and pneumonectomy was done in one case. The rest
2 cases were inoperable because of superiot vena caval syndrome and frozen hilum.
8. It seemed that the bronchial obstruction and ischemic necrosis were the cavitify mechanisms
of pulmonaary cancer.
